Transcript Request Form

Students and graduates of HTS who wish to receive a copy of their transcript, whether official or student's copy, should fill in the online request form on this page.  The Seminary provides paper transcripts or an e-transcript option.  E-transcripts are provided at no charge, while paper transcripts are $5, or more for expedited mailing.

Transcript requests made using a paper form are still available.  Please download and print a PDF document with the offline Transcript Request Form and send a completed Transcript Request Form by regular post only, with accompanying payment, to Holy Trinity Seminary, attn: transcript request, P.O. Box 36, Jordanville, NY 13361.  Transcript requests using this form and sent by email or fax will not be processed.
